Licensed Practical Nurse/LPN, Part Time, Afternoon Shift, Greenfield, WI  


Community Care is hiring Licensed Practical Nurses/LPNs who are committed to teamwork and providing great care to become a part of our compassionate caregiving team. The Licensed Practical Nurse/LPN provides nursing care and services for our residents. This is a part time opportunity on our day shift for Our Lady of Angels location at 3995 South 92nd Street, Greenfield, WI 53228. 


Licensed Practical Nurse/LPN Job Responsibilities 

  • Work in collaboration?with other nursing staff to coordinate and provide direction of care during weekday operations.  

  • Serves as coordinator of care for scheduled weekend and holiday shifts.  

  • Ensures care and service delivery are in conformance with regulations, policies, procedures, and standards, established by Community Care, the Program for all-inclusive Care for the Elderly and the State of Wisconsin. 


Licensed Practical Nurse/LPN Job Requirements:  

  • Education:?Diploma from accredited program in practical nursing required. 

  • Licensure: A current Wisconsin Licensed Practical Nurse/LPN license in good standing 

  • Certification: Current CPR/AED certification 


Licensed Practical Nurse/LPN Job Schedule: 

  • 6:30 AM – 2:30 PM 8 hours per week 

  • Every other weekend 

  • Every other holiday
