The Advanced Practice Nurse independently provides clinical case management, theory research-based care to clients facilitating attainment of health goals, works with the multidisciplinary staff to advance clinical practice to improve outcomes and provide clinical expertise to affect change to improve quality of care. Responsible for coordination of patient care from admission through discharge.
Requires a BSN MSN, licensure to practice in the State of Wisconsin and certification within clinical specialty area. Must demonstrate excellent verbal written communication skills and strong leadership organizational skills. Need to be familiar with nursing computer systems PCs. Certification of an accredited Case Management association required within one year of employment.
