Job Description

Responsible for arranging necessary healthcare services for patients requiring nursing services in their homes after normal hours of operation, which include case management, including conducting initial assessments, establishing a plan of care for patients, and providing direct patient care. Responsible for all activities related to on-call visits and communicating changes to the primary Registered Nurse. Evaluates and executes appropriate treatment for referred patients with emphasis on quality of care, continuity of services, and cost-effectiveness.

General Qualifications

Valid WI RN License
Valid WI Drivers License, insured vehicle, good driving record.
2 years of RN experience in acute care setting or in home care/hospice

Desired Qualifications

Previous home health/hospice experience.
Symptom management Experience
