Job Description

Imagine yourself working with this team… The Care Management Department is part of an integrated system where quality is our #1 Priority. Imagine yourself serving as an internal consultant with a broad working knowledge of the health care industry, performance improvement, and the use of clinical data to coordinate the development, implementation and support/maintenance of organization-wide clinical improvements. Imagine yourself… *Identifying and applying appropriate improvement theory and framework to project: charter, comparative data, goals/aims, measurement strategy, and change package.
*Coordinating project planning to include team selection, orientation to roles, facilitating engagement and coordination of pre-work. Assuring that activities of team support organizational strategic objectives. Monitoring the project (team progress, timelines, resource allocation), taking action required to successfully meet project goals. Serving as key communication link between team members, and between various sub-teams and leadership. Acting as a liaison to facilitate collaboration and problem solving between team members and diverse groups.
*Teaching and coaching teams on application of improvement theory and framework.
*Supporting implementation of organizational-wide clinical improvements.
*Acting as a liaison with key internal stakeholders, as it pertains to achieving our strategic goals.
*Collaborating in the design of the measurement system and addressing issues regarding measurement. Assisting with interpretation and analysis of data.
*Participating in the application, development and implementation process of grant related work. Collaborating with external agencies to further our clinical initiatives.



Skills / Requirements
General Qualifications

*Knowledge of quality improvement principles, project management, and team facilitation, equivalent to that which would be acquired by completing a regionally accredited bachelor's degree program in a health care field.
*Knowledge, skills and abilities required to perform this job are typically acquired through 3 - 5 years of progressively responsible quality improvement experience in a clinical setting, including facilitation of multi-disciplinary teams, project management, and developing recommendations based on data interpretation.
*Previous experience with the methods of Quality Improvement.
*Working knowledge of JCAHO and/or NCQA regulations and standards.
*Understands medical staff bylaws and rules and regulations.
* Excellent written and verbal communication, planning and problem solving and decision-making skills.
*Demonstrated ability to work collaboratively and in partnership for effective and efficient accomplishment of objectives and tasks
*Demonstrated project planning and project management skills, excellent organizational skills, ability to prioritize and organize many competing tasks, good problem solving and decision-making skills.
*Demonstrated ability in the use of computers and computer software.
*Demonstrated ability to work with multi-disciplinary teams and other professionals in a direct, honest and positive manner.


Desired Qualifications

*Minimum one (1) year of quality management experience in clinical setting preferred.
*Previous experience in statistical analysis is strongly preferred preferred (includes interpreting data and making recommendations based on that data).
