Job Description

The Geriatric Bridge Program, in partnership with the Geriatricians at Aurora Sinai Medical Center are seeking a Geriatric Nurse Practitioner to see highly vulnerable patients from the Racine and Kenosha areas in a clinic environment.

Due to our commitment to shared governance and shared decision-making, the Nurse Practitioner, in association with a designated physician or group of physicians, assists in the delivery of health care services and is responsible for the patient assessment, interpretation, diagnosis, and prescribing of appropriate medical and nursing therapeutic measures. .

Eliciting a personal medical history, performing physical exam, and assessing and determining differential diagnosis. Prescribing accordingly.
Consulting with physician as appropriate. Performing routine therapeutic procedures including, but not limited to, injections, immunizations, lesion destruction/removal, suturing and care of wounds.
Instructing and counseling patients on physical/mental health including, but not limited to, diet, preventive health measures, disease, treatment, and normal growth and development.
Independently performing evaluation and treatment procedures necessary

General Qualifications

Board Certified Adult Nurse Practitioner - Specialization Geriatrics
Experience in managing high risk elder patients both in hospital and clinic setting
Ability to work collaboratively as a team member with Senior Resource Nurse, Geriatrician and Referring Providers as well as NH/Assisted Living Staff, and family members
Solid knowledge of Advanced Directives, Palliative Care Guidelines, CHF, COPD, Falls Prevention,
Able to speak comfortably with patients and families about their healthcare
Excellent assessment skills and utilization/understanding of geriatric assessment tools - medical and cognitive
High level of knowledge of geriatric syndromes
Identifying and differentiating normal and abnormal conditions
Able to select appropriate treatments
Strong knowledge of the appropriate treatment referrals and or consultation/care coordination
Able to case manage/monitor patient outcomes
