*Being a member of the case management team and aiding the patient and their families with personal and environmental difficulties that affects their discharge plans and/or smooth transition to the next level of care.


This position is a casual position, with zero assigned/guaranteed hours, and will be used on an as needed basis.

General Qualifications

Master of Social Work degree and Wisconsin certification as a social worker.

At least one year of experience as a medical social worker.

Desired Qualifications

Hospital experience in discharge planning and case managerment.
