Job Description

Our Visiting Nurse Association has been finding better ways to keep our patients independent in their homes for 100 years. Founded in 1907, the Aurora VNA serves people of all ages throughout eastern Wisconsin regardless of their ability to pay.

Imagine yourself &-
*Assuming primary responsibility for a patient caseload.
*Communicating with members of the interdisciplinary team to promote continuing coordination of services.
* Conducting a physical therapy evaluation of each one of your patients and establishing a plan of care and treatment goals.
*Communicating with physicians to confirm, correlate and update patient's plan of treatment and providing physician with an update on a regular basis.
*Teaching appropriate treatment procedures and supportive activities to patients and caregivers, and developing a plan for obtaining adaptive devices that your patient needs.

General Qualifications

*A minimum of a Bachelor's degree in Physical Therapy.

Wisconsin State Licensure required.

*Must obtain CPR certification within three months of hire.

*Excellent time management and multi-tasking skills, organization, problem solving, and decision-making skills required.

*The ability to work self-directed and as a team.

*Customer service skills are essential.
*A Valid Wisconsin driver's license is required

Desired Qualifications

*Must be able to work independently in their patient care and scheduling.
*Must have good clinical skills and knowledge in Geriatric care and knowledge of a variety of Medical Conditions.
*Experience in home care preferred
