Job Description

Imagine yourself working with a multi disciplinary therapy team and other professional medical and office staff. Imagine working with a team that is the perfect size to provide the resources and support you need! Imagine yourself assessing patients and determining he appropriateness of inpatient rehabilitation programs.

Are you looking for a position to help supplement your income? We are looking for an individual that is interested in working a part time position every other weekend with some weekday coverage available if desired. This position is located at Aurora St. Luke's Medical Center. Imagine Yourself &-

*Coordinating patient admissions in the inpatient rehabilitation unit following established admission criteria, Medicare guidelines and other insurance carrier criteria, and working in conjunction with other Referral Coordinators to facilitate admissions to other inpatient rehab sites when needed.

*Performing pre-admission assessment, on-site and by phone, of the patient to determine if, and when, inpatient rehab is appropriate and would benefit the patient by evaluating the criteria from the medical chart, communication with care providers, physicians and patient/family, and input from other stakeholders.

*Having daily contact with physicians regarding admission decisions, and performing insurance verification for the accepted candidates.

*Educating clients, family members, referral sources, hospital personnel, external payers, and external case managers as to the rehab program benefits and services. As well as serving as a consultant with attention to rehab potential, therapy progress, discharge plan feasibility, and special equipment needs.

*Maintaining referral sources by developing and maintaining positive relationships with referring physicians and other referral sources, and maintaining accurate documentation on admissions, discharges, and referrals.

* Being responsible for portions of the IRP Program Evaluation system and coordination of the follow-up process, which includes making phone calls to patients, data entry, and date analysis.

*Working as a liaison with the internal and external case management team to provide a cost effective plan for each client while providing ongoing monitoring and analysis of trends, issues and data to consistently evaluate service quality, communication systems, and program use.


Skills / Requirements
General Qualifications

*Current licensure as a Registered Nurse (RN) granted by the State of Wisconsin,
*A minimum of two years clinical experience working in an inpatient rehabilitation program.
*Must be available for off-site travel as needed and excellent communication and human relations skills are essential.
*Basic word processing and computer skills required.

Desired Qualifications

*Extensive experience in interacting with physicians and other rehab staff.
*Must possess the ability to work independently
