Home Care Case Manager

Coordinating and providing care for your patients. Sound interesting? We are looking for an
Experienced, professional, Registered Nurse, 2 yrs in Geriatrics minimum requirement, to work full time, one to one with our skilled, med management and supervisory visit patients. Must have good interpersonal skills, be able to problem solve, and document accurately and comprehensively.
