We focus on maintaining quality of life by keeping patients pain-free and comfortable while providing for spiritual, emotional, social and physical needs. Home care practice allows you personal flexibility for work-life balance by setting schedules that balance patient needs with your own personal priorities. The home care setting is very rewarding and challenging, allowing you to get to know your patients in a very personal setting and to be creative in meeting their needs. * Assumes primary responsibility for a patient caseload. Coordinates all services and collaborates with members of the interdisciplinary team to provide consistent patient care. * Conducts initial OASIS assessment for Palliative Care patients and establishes plan for treatment to obtain thorough physical, psychosocial and environment data; establishes and follows a care plan which identifies problems, (nursing diagnosis) and evaluates outcomes within a certain time frame. * Performs the initial hospice admission assessment and establishes the initial Plan of Care (POC). * Communicates with physicians and other members of the Interdisciplinary Team to confirm, correlate and update patient plan of treatment and provides physician with a status report on a regular basis. * Provides on-going assessment of patient response to treatments, medications and teaching while making appropriate changes in interventions and follow-up to meet patient needs. Updates and revises POC as indicated with supportive documentation to changes. * Utilizes Care Management Initiative material consistently where appropriate and available. * Participates in regularly scheduled Interdisciplinary Team Conferences to identify and discuss a holistic approach to management of patient/family goals of care and desired outcomes. * Instructs paraprofessional staff on a timely basis and in accordance with agency, state and federal regulations. * Facilitates the discharge planning process according to agency standards and utilizes interagency/system and community resources to assure continuity of care after discharge. * Accurately maintains and updates the patient's clinical records according to agency, state and federal guidelines. * Communicates problems and changes in condition to physicians, supervisor and other members of the Interdisciplinary Team making appropriate changes to the patients POC to accurately reflect current status of the patient and continued goals for care and treatment. Skills / Requirements
General Qualifications
* Current Registered Nurse (RN) license granted by the State of Wisconsin. * Knowledge of Nursing, equivalent to that which would be acquired by completing a regionally accredited associate's degree program. * Knowledge, skills and abilities required to perform this job are typically acquired through a minimum of two years of progressively responsible experience in direct patient care and in Home Health/Hospice, Medical/Surgical, or Oncology nursing. * Proficiency in clinical, communication and organizational skills with the ability to work independently and make sound judgments. * Ability to perform all of the essential functions of the job with or without accommodation. Desired Qualifications Minimum of 2 years nursing experience. Hospice and/or Home care experience desired.
